CLAIM INVESTIGATION REPORT FOR LIC’s CANCER COVER POLICY

INSTRUCTIONS TO THE CLAIM INVESTIGATOR
The investigator has to ensure that:-
i) There is no non-disclosure as per the details given below:-
a) The life assured having suffered from reported cancer before the date of proposal/ date of policy or date of revival.
b) The cancer was diagnosed during the period of three years from the date of policy/revival.
c) The life assured having suffered from illness/complaints before the date of commencement of policy/ date of a revival led into the present contingency.
ii) The identity of the life assured is to be established by ensuring that the reports
submitted pertained to him only, by making enquiring with the usual medical attendant/
specialists.
iii) In respect of hospital reports received from the life assured the authenticity of the
reports including the existence of the hospital/ medical center should be ascertained.
